Wag Away, LLC - Basic Training & Liability Agreement 

The following is a liability agreement and terms for Wag Away, llc.  Please read and initial each point carefully and sign the bottom.  Thank You!

_______Wag Away LLC is not responsible for any liability of both the dogs and owners during training.  This includes but is not limited to bites, scratches, dog fights, loose dogs, any injury to the dogs or humans, or any damage caused by or to the client’s dog.  Should any incident occur it is the sole responsibility of the dog owner to take care of all factors surrounding the incident.

_______Wag Away LLC is not responsible for paying any medical or veterinary bills should damage or the dog in training, other dogs, pets, other people or the owner occur during a tanning session or after a training session.

_______Wag Away LLC is a service meant for dog training and should not be solicited for any other service including pet sitting, pet grooming or house services.

_______ I understand that Wag Away LLC requires at least 3 sessions to book (excluding package sessions that already include 3 sessions).  All sessions will be scheduled at the first session.  Any subsequent sessions are solely the choice of the client and can be scheduled any time after. 

_______ I understand that 50% of payment is due upon booking a private session/package or a group class with Wag Away LLC and total payment due at first class.

_______ I am aware that there are no refunds once sessions are booked and after sessions have passed.  Once booked, you are responsible for showing up for each training session.

_______ I understand that anytime sessions run over time will be an additional $15 for 15 minutes.

_______ I understand that it is the sole responsibility of the owner to attend classes/training they are registered for or to contact the instructor if they can no longer attend.

_______ I understand that any training package that are purchased have a 60-day expiration date.  Any     additional training sessions that are purchased have a 30-day expiration date AFTER payment rendered.

_______ I am aware that training is a lifelong commitment.  Progress made between or after sessions is the sole responsibility of the owner.

_______ Photo Release: I, the undersigned/Client, do hereby consent and agree that WA, LLC has the right to take photographs, videotape, or digital recording of my pets and to use these in any and all media. Now or hereafter known, and exclusively for the purpose of marketing and business development.

This agreement is made and entered on, ____ by and between Wag Away, LLC of Granby, CO (hereinafter referred to as “WA, LLC”) and Client, _________, (hereinafter referred to as “Client”) residing/physical address: ____________(hereinafter referred to as “Premises”)

Service/Fees/Cancelation:

1. Business hours fall between: Dawn to Dusk Monday-Saturday .  Services are usually completed during these hours, but can be flexible if needed.
2. To guarantee a service commitment for your pet, total cost of pet care is due at time of initial booking and is non-transferable/non-refundable.  This includes HOLIDAYS __________(Int)
3. On Holidays, an additional $30 charge will be added to that day’s pricing.  All charges are non-refundable/non-transferable  _______(Int)
4. A handling charge of $70 or half the total amount (whichever is greater) will be added to all RETURNED checks. ______(Int)
5. Refund and Cancellation Policy: There are NO refunds if the client cancels or comes home early.  _____(Int)
6. If the client plans to lengthen their trip they must contact WA,LLC 12-hours in advance of the last pet care day in order to be put back on the schedule. The owner agrees to pay the additional fee incurred promptly upon their return. _________(Int)
7. If another is staying at clients’ home (adult, housekeeper) while the client is out of town, WA,LLC can NOT be held responsible for care of pets outside of visit.  ______(Int)
8. If WA, LLC agrees to accept a pet/client, all pets must be walked on a leash. Times of service have a 2-hour window on each side of agreed time. _____(Int)   ______(Int Off Leash)
9. It is up to the client to provide appropriate working equipment. ______(Int) 
10. Any malfunction of pet equipment, WA, LLC is not liable if the animal escapes. _________(Int)

Medical:  

1. The client understands that all pets (where appropriate), must be current with all vaccinations.  Including: Rabies, Distemper & Parvo _____ (Int)
2. WA, LLC will not be liable if your pet bites or attacks another animal/person while in our care. This includes expenses if your animal is attacked by another animal while in our care. ________(Int)
3. WA, LLC reserves the right to take pet(s) to the nearest veterinarian. The client is responsible for all charges. _______(Int)
4. VET: I authorize WA, LLC employee/independent contractor to act as my agent in the event that my pet needs medical attention.  I further agree that I (the client),will be responsible for any and all costs of any veterinary care deemed necessary  by the licensed veterinarian.

Weather:

1. Due to ever changing weather in Colorado, WA, LLC reserves the right to do a shorter walk or quick outdoor potty break but will stay and play/groom for an amount of time contracted. ____(Int)
2. WA, LLC reserves the right to “increase the rate for “time spent in the car,” due to road conditions and weather. ____(Int)
3. WA, LLC will carry liability insurance. A copy of the insurance policy is available on the website and upon request.  ____(Int)
